Macrophage activation syndrome (MAS), a form of secondary hemophagocytic lymphohistiocytosis (sHLH), is a life-threatening hyperinflammatory complication of systemic autoimmune diseases (especially sJIA, adult Still's, SLE, Kawasaki) affecting ~10% of sJIA patients; driven by NK cell and CD8+ CTL perforin/granzyme B dysfunction (PRF1/UNC13D/RAB27A mutations), uncontrolled macrophage M1 activation, IL-18 (often >10,000 pg/mL) and IL-6/TNF-alpha cytokine storm, hemophagocytosis in marrow/spleen/liver, ferritin >500 ng/mL (often >10,000), NLRP3/caspase-1/IL-1beta cascade, JAK1/2-STAT1/STAT4 signaling, and disseminated intravascular coagulation (DIC). Sea moss fucoidan modulates NF-kB, NLRP3, JAK/STAT complement cascades; selenium supports macrophage GPx1/GPx4 against ROS in cytokine storm; omega-3 modulates M1/M2 macrophage arachidonic acid eicosanoid balance and resolvin D1; zinc supports FOXP3+ Treg and NK/CTL functional restoration. Cyclosporine, etoposide, anakinra (IL-1 blocker), ruxolitinib (JAK inhibitor), and emapalumab (anti-IFN-gamma) are established treatments.
