Bullous pemphigoid (BP) is the most common autoimmune subepidermal blistering disease, primarily affecting adults over 70, caused by IgG autoantibodies against BP180 (type XVII collagen) and BP230 in hemidesmosomes at the dermal-epidermal junction, triggering complement C3/C5a-mediated eosinophil and neutrophil degranulation with elastase/MMP-9 dermal-epidermal cleavage. BP involves IL-5/IL-4/IL-13 Th2 eosinophilia, NF-kB amplification, and eotaxin-driven eosinophil recruitment.
