IgA nephropathy (Berger's disease) is the most common primary glomerulonephritis worldwide, affecting ~2-3 per million annually, caused by galactose-deficient IgA1 (Gd-IgA1) immune complex deposition in the glomerular mesangium, triggering complement C3/C4b-mediated mesangial proliferation, NF-kB amplification, TGF-beta fibrosis, and progressive CKD.
