Eosinophilic granulomatosis with polyangiitis (EGPA, formerly Churg-Strauss) is an ANCA-associated vasculitis affecting ~13 per million, characterized by asthma, eosinophilia, and necrotizing granulomatous small vessel inflammation of lungs, skin, peripheral nerves, GI tract, and heart. EGPA involves Th2 IL-4/IL-5/IL-13, eosinophil-derived major basic protein (MBP), peroxidase (EPO), and NF-kB inflammatory cascades. Sea moss omega-3 EPA/DHA modulates Th2 eicosanoid LTC4/LTE4 production; fucoidan modulates NF-kB and eosinophil recruitment; selenium supports eosinophil GPx and cardiac protection; zinc supports Treg regulation. Prednisone, mepolizumab (IL-5 antagonist), and cyclophosphamide are standard; sea moss provides anti-inflammatory nutritional support.
