Susac syndrome (retinocochleocerebral vasculopathy) is a rare autoimmune endotheliopathy (~1/million) targeting small vessels of the brain, retina, and cochlea; driven by IgG anti-endothelial cell antibodies (anti-ESAM/CLDN5/CD166), CD4+ Th1/Th17 perivascular infiltration, complement C4d endothelial deposition, VEGF-A upregulation causing blood-brain/blood-retinal/blood-labyrinth barrier breakdown, NF-kB/IL-6/TNF-alpha endothelial activation, and microthrombi formation in end-arterioles causing branch retinal artery occlusions (BRAO), sensorineural hearing loss (low-mid frequency), and corpus callosum 'snowball' demyelinating lesions on MRI. Sea moss fucoidan modulates NF-kB, complement C4d, and endothelial VCAM-1/ICAM-1; selenium supports endothelial GPx1/GPx4 and reduces oxidative stress; omega-3 modulates vascular PGI2/TXA2 eicosanoid balance and resolvin D1 anti-inflammatory resolution; zinc supports ZO-1/claudin-5 vascular tight junction integrity. High-dose corticosteroids, IVIG, cyclophosphamide, rituximab, and mycophenolate are standard; anticoagulation for microthrombi.
