Autoimmune retinopathy (AIR) encompasses non-paraneoplastic AIR (npAIR) and cancer-associated retinopathy (CAR/MAR); driven by IgG autoantibodies against photoreceptor proteins (anti-recoverin/RCVRN, anti-carbonic anhydrase II/CA-II, anti-alpha-enolase/ENO1, anti-TRPM1 in MAR, anti-CNGB3), complement C3/C4 retinal deposition, CD4+ Th1/Th17 infiltration of the retinal pigment epithelium (RPE), NF-kB/IL-6/TNF-alpha inflammatory cascade, VEGF-A outer retinal barrier disruption, and photoreceptor apoptosis via complement MAC and caspase-3/7 activation causing progressive visual field loss, electroretinogram (ERG) amplitude reduction, and night blindness. Sea moss fucoidan modulates NF-kB, complement C3/C4, and retinal vascular NF-kB/IL-6 cascades; selenium supports RPE GPx1/GPx4 and photoreceptor antioxidant defense against complement-mediated ROS; omega-3 DHA constitutes ~50% of photoreceptor outer segment membrane phospholipids and modulates resolvin D1/D2 anti-inflammatory resolution; zinc supports rhodopsin/carbonic anhydrase metalloenzymes and FOXP3+ Treg. IVIG first-line for npAIR; rituximab/mycophenolate for refractory; anti-VEGF for macular edema.
