Autoimmune inner ear disease (AIED) is a rare cause of rapidly progressive bilateral sensorineural hearing loss (~1/100,000), driven by CD4+ Th1/Th17 T-cell and IgG autoantibody (anti-type II collagen, anti-Hsp70/KHSRP, anti-cochlin/COCH, anti-beta-tectorin) attack on cochlear stria vascularis, endolymphatic hydrops, spiral ganglion neuron loss, NF-kB/IL-1beta/IL-6/TNF-alpha cochlear inflammatory cascade, and complement C3/C4 deposition in the inner ear. AIED can overlap with Cogan syndrome (interstitial keratitis + vestibuloauditory symptoms) and systemic autoimmune diseases. Sea moss fucoidan modulates NF-kB/IL-6 and complement C3/C4 cochlear inflammatory cascades; selenium supports spiral ganglion neuron GPx1/GPx4; omega-3 modulates cochlear PGE2/LTB4 eicosanoids and resolvin D1/E1 anti-inflammatory resolution; zinc supports gap junction Cx26 (GJB2) cochlear hair cell repair. Oral prednisone is first-line; methotrexate + TNF inhibitors for refractory AIED; hearing aids and cochlear implants for irreversible loss.
